Freshmen & Transfer Student-Athlete Forms


Prior to participating in a sport at St. Norbert College, student-athletes must provide the athletics department with a current address, plus emergency contact, insurance, medical alert and health history information. To expedite this process, sports medicine services uses the online data entry system Healthy Roster. Student-athlete information is collected annually, and must be completed in summer before reporting to St. Norbert (deadline is Aug. 1). Student-athletes will not be allowed to participate in any team activity until these forms are on file.

These forms are completely separate from, and should be treated separately from, any forms required by St. Norbert College Health Services. Do not send forms with the heading "Intercollegiate Athletics" to Health Services. Do not send forms with the heading "Health Services" to the athletics department.

Required Student-Athlete Forms/Information

Freshmen and transfer student-athletes must complete the following:
  • Pre-Participation Physical Exam Form 2025-26
    • Student-athletes only need one physical during their careers at St. Norbert College. Physicals from high school are not acceptable; a new and up-to-date exam is required and must use the provided form, signed by a physician. Sickle cell trait information is here.
  • Healthy Roster - You will receive an email invitation titled "Join St. Norbert College on Healthy Roster."
    • Once you create your account, there are seven electronic documents that you will need to complete in Healthy Roster.
  • ImPACT Baseline Assessment
    • Once your coach provides the Sports Medicine Staff with your information, you will receive an email at your St. Norbert account to take your ImPACT.
  • Acknowledgement of Insurance Requirements Form and Electronic Letter
    • This digital form must be completed by a parent, guardian or legal representative of the student-athlete each year.
  • Sports Information Questionnaire (please use online form)

For athletes diagnosed with ADD/ADHD only:

  • NCAA Reporting Form for ADD/ADHD Medications 
    The athlete's physician must complete the NCAA reporting form and include supporting documentation. This form needs to be completed only once, unless the prescribed medication changes.
